Extremes Questionnaire (X-Q)
The X-Q is a screening inventory used to get an idea of the amount of chaos in your life. Higher numbers may indicate a need for consultation to determine psychotherapy or coaching is warranted. The X-Q is not meant to diagnose or treat any condition and is to be used as a an estimate of the amount and effects of chaos.
Think of the last six months as you complete the inventory.

Be honest. Don’t minimize.
